Review/debug/docs state: PR #172 retains the reviewed implementation; no runtime debugging phase was needed; docs ingest is complete.

Separate operator actions:

1. Human operator runs hi ensure and selects the approved top-level Linear Root Initiative.
2. Human reviews and explicitly approves the exact provider topology preview.
3. Live initialization, destination migration, and Normalization remain explicit operator actions; delivery-state transitions remain evidence-gated runtime operations.
